A Funeral Planning Declaration Form was created by the Commonwealth of Kentucky Office of the Attorney General. This allows a person being at least eighteen (18) years of age and of sound mind, willfully and voluntarily make known their instructions concerning funeral services, funeral and cemetery merchandise, ceremonies, and the disposition of their remains after their death. In order for the document to be valid, an individual must put their wishes in writing on the form. The form needs to be signed and dated, and witnessed by two individuals. Your signature must also be acknowledged by a notary public.
