Jeremy John Dworshak, 40, of Radcliff, KY passed away Thursday, June 22, 2017 at his residence.

He was preceded in death by his father, Gerald Dworshak.

Survivors include:
His mother, Karen Dworshak; two sisters, Jessica Owens, Rachel Kaelin & her husband, Travis; four nieces & nephews, Christopher, Kaitlyn, Angelina, Gerald; and numerous aunts, uncles, cousins & friends.

Visitation will be from 9:30 AM until 11:00 AM Tuesday at Coffey & Chism Funeral Home.

A Prayer Service for Mr. Dworshak will be held at 11:00 AM Tuesday, June 27, 2017 at the chapel of Coffey & Chism Funeral Home, Vine Grove, KY. Burial will follow at the St. Brigid Cemetery, Vine Grove, KY.
